Dr. Barnard describes the science behind the causes/triggers that create type 2 diabetes and insulin insensitivity, both in his books, and in his speaking engagements around the U.S. and around the world, as well as through the NIH-funded (National Institute of Health) scientific studies investigating the causal link between nutrition and diabetes that Dr. Barnard has led beginning more than 15 years ago.
To paraphrase in a nutshell (paraphrased here in layman’s terms), Dr. Barnard describes how type 2 diabetes occurs as a result of an excess of fats (eaten in foods) building up at a (microscopic) cellular level inside the muscle cells (intramyocellular lipids) and inside the liver cells (intrahepatic lipids).
When these tiny particles of fat (intramyocellular lipids and intrahepatic lipids) build up inside the muscle cells and inside the liver cells, the fat (lipids) block insulin’s ability to transport glucose into the cells. The cells need glucose (which is stored as glycogen) for energy and for basic essential functions including regulating body temperature.
Since the insulin is blocked by intramyocellular lipids from transporting glucose into the cells, blood sugar builds up in the blood, but is not reaching the body’s cells — including brain cells! — that need the glucose/glycogen to function optimally.
This leads to feelings of brain fog (since the brain is not getting the glucose/glycogen fuel it needs) and tiredness, weakness (muscle cells not getting the glucose/glycogen fuel they need for even basic baseline activity.)
And as any diabetic can attest, that tired/low-energy feeling only scratches the surface of the havoc wreaked upon the body by diabetes.
Since this is such an important topic in understanding how to reverse diabetes, read Dr. Barnard’s description below, in his own words, (quoted from a recent interview) about how type 2 diabetes happens as a result of the foods people choose to eat (or, for children with type 2 diabetes, the foods their parents/caregivers – unwittingly — choose for them).
Dr. Barnard recently described how type 2 diabetes happens and how to reverse diabetes in a recent interview videotaped for the Plant Fit Summit 2018:
[F]or some reason [some] people have really missed the science, that has been out there for 15 years or more, which is as follows: that when you eat fatty foods, the fat can enter the muscle cells and the liver cells, as tiny particles of fat called intramyocellular lipid in the muscle cell, or intrahepatocellular lipid in the liver cell. They stop insulin from being able to work. They gum up the cell if you want to put it that way, so the insulin can no longer bring sugar into the cell.
And the answer is to get the fat out of the diet. . . . A vegan diet has no animal fat at all, and if [you] keep the vegetable oils really low, we then believe that fat inside the cell [dissipates], and the insulin resistance will go away, and the diabetes has a chance of going away too, especially if you catch it early.
Now, [for] the people who say ‘Well, when I avoid sugar I do better,’ it’s true that if your cells are full of fat and they can’t take in sugar, then to the extent that you avoid sugar and you avoid carbohydrate, your blood sugar will not go up so much.
But you need to fix the cause of it, and the cause of it is this build-up of fat, especially animal fat, but vegetable oil can contribute too, and you want to get that out of your cells.” (Plant Fit Summit 2018 Dr. Neal Barnard interview, at 5:31, emphasis added.)
Dr Barnard continues:
[When you switch to a whole food plant-based low-fat diet], what we believe is happening is the fat that is inside the cell starts to dissipate, and then the insulin which your body is making anyway … will attach to the cell, and it will allow the blood sugar to come down. The sugar in the blood now can go into the cell more easily because [much of] that fat is gone. And that fat is the cause of the insulin resistance which is causing the disease. So when a person goes on a low fat vegan diet, what they discover is sometimes their blood sugar falls REALLY fast, even if they are eating a lot of carbohydrate.
Their blood sugar will fall, and that’s because their insulin resistance is going away, their insulin sensitivity is returning. And that can happen within the first week in some cases. They lose weight, their cholesterol falls, their blood pressure falls, their need for medication goes way, way down, their energy comes up.
. . . .
[E]verybody is different; if you catch this disease early, in some cases it just goes away. If a person has had diabetes for 30 years, it’s not necessarily going to go away, it may well not, because they’re not producing as much insulin as before, but nonetheless this is still the kind of diet they want to be on, because it protects their arteries that would be assaulted by the disease. You don’t want any cholesterol. You don’t want any animal fat in your diet, you want to protect yourself.” – Dr Barnard interview, 2018 Plant Fit Summit at 5:52 to 7:25.
Dr. Barnard also notes the dramatic weight loss benefits of switching to whole food plant-based nutrition (low- or no-added-oil): “[T]he proof of it is [clear when people] come into our clinic, and we put people on a low fat vegan diet, and they lose weight like crazy. They are not building more body fat [by eating whole food plant-based carbohydrate/fiber-rich, no-oil-added foods.]”
. . . .
Until fairly recently, people thought [type 2 diabetes] was irreversible. We now know that [type 2] diabetes isn’t so much a diagnosis that you get and you keep, so much as it is a state that you get in, and it can change. It can get worse, it can get better, and in certain cases, it can go away.
[With healthy whole food plant-based low-oil nutrition,][for] a LOT of people, particularly if you catch this early, they can tackle this disease and it can go away, and they can live a totally normal life.
I’m very excited about this.”
Dr. Barnard concludes:
I am happy to say that convention is changing, but when we got started in all of this, the conventional diet was cutting calories to help people lose weight, because most people with type 2 diabetes needed to lose weight … And the idea was well, just eat less. And that wasn’t so hot, because if you give a person a menu that just cuts out a lot of foods, by Wednesday they want to eat the sofa, because people don’t want to be hungry.
So instead [we teach people to reverse diabetes through a whole food plant-based diet] that fills you up with high fiber healthy foods that are satisfying so you don’t have the hunger.” Dr. Neal Barnard, 2018 Plant Fit Summit.
When asked what percentage of people succeed in reversing type 2 diabetes in percentage, Dr. Barnard clarified that,
it depends entirely on how long the person has had the disease, and how well they [commit to whole food plant-based low- or no-added-oils nutrition]. If you get a person who is just tipping into the disease, I think in virtually every case it can be reversed (when I say reversed, I mean go away with no medication).
But on the other hand, most people at the time they are diagnosed have had the condition for five years or more. And at that case their body has been hurt by it. The pancreas has beta cells that make insulin, and they have been beat up by this disease.
And the longer this goes on, the harder it is for them to recover. . . . So get to it early.”

If a person eliminates animal products, [first] take a week, figure out what you like to eat that’s free of animal product. [For example, oatmeal with blueberries or strawberries on top.] Figure out what these foods are, and as time goes on, even though you aren’t intentionally cutting calories, by choosing low fat, high fiber foods you are getting full sooner.
[By eating this way] you aren’t thinking about it, you aren’t trying to lose weight; it just happens and it can be very significant.
The average person loses between half a pound to a pound a week. With 52 weeks in a year, [the average weight loss by switching to whole food plant-based low- or no-added-oils nutrition] is between 26 and 52 pounds per year. A person can easily lose 20 Kg in course of a year, that’s not uncommon at all.” – Dr. Neal Barnard, 2018 Plant Fit Summit interview.
Dr. Barnard also points out that the benefits of exercise apply whether you walk or whether you run, so starting out on your weight loss and diabetes reversal journey, do what you can to get some level of exercise in. Exercising muscle pulls sugar out of the blood with very little insulin. Dr. Barnard notes that studies show that exercise affects the brain, improving memory. However Dr. Barnard reiterated that when people exercise, they need fuel. Exercise works with diet, but it doesn’t take place of a healthy whole food plant-based (low- or, ideally, no-added oils) diet.
